At the top of the form:

• Werkzaamheden uitgevoerd bij: Customer/owner name
• Naam schip: Name of the ship
• Naam werknemer: Your first and last name
• Functie: Your position on board
• Weeknummer: Calenderweek
• Tijd aan boord: Time you board
• Tijd van boord: Time you when you leave the ship

Duur van de werkzaamheden: Working Time (if necessary, divide across multiple forms):

• Datum: Date of the relevant day
• Uren: The number of hours worked
• Kilometers: Kilometers travelled by your private car (not rental cars)
• Declaraties: Costs for e.g. groceries (attach sales slips)
• Opmerkingen: Any comments that are important to our administration

On the bottom of the form:

• Akkoord werknemer: Your signature
• Datum: Date of departure
• Akkoord leidinggevende: Signature of the captain or owner of the ship with company stamp
• Datum: Date of departure

You will mention them on your registration form and you send the original vouchers (sales slips) together with your form to our offices.

You are not able to go or stay aboard? Then inform the captain, Inland Shipping SA and the CNS (Caisse Nationale de santé) immediately!

The sick report must always be confirmed in writing with the form E115 or E116 completed by your doctor.
Please send the original sick certificate to the CNS (IMPORTANT: your matriculation number (social security number) must be indicated on the back of the sick certificate) and a copy to Inland Shipping SA.

It is important that you are within the authorities’ reach during your illness and cooperate with the procedure. There might be check-ups at your home address!

!!PAY ATTENTION!! If you do not follow this procedure, you are not ill according to Luxembourg law. Absence at work due to illness, must be attested in Luxembourg. If the necessary forms have not been provided, you are officially not ill and thus taking free time. This means in practice that you will not be paid a salary!

You need your social security number (matriculation) to renew or extend a European Health Insurance Card. This application is made by Inland Shipping SA the first time, after which you have to make sure that a new one is requested 30 days before the card expires. Please visit the following page:

Here you will find the form to request a new card. In the first field enter your 11-digit social security number (matriculation: 19 … always begins with your date of birth). Insert the security code and send (Envoyer)

Form S1 will be requested by Inland Shipping after signing the contract and will be sent to your home address. You must forward this to your health insurance. Please ask your health insurance if you need to send the original document or if you can send it as an attachment by e-mail.

The tax card will be sent to Inland Shipping after the company has registered you. If it is sent to you directly by mistake, you need to send it to the offce in Remich immediately. If you do not send it, you will not be payed correctly and will receive less money!

You will obtain them from the administration in Remich.
